GENERAL CASE PLANNER POSITIONS (Bi-lingual preferred)

Summary of Position

Ideal candidate will be responsible for all aspects or elements of casework services, e.g., assessment, individualized service planning, case planning/reviews, child-family engagement and referrals and case conferences. The incumbent defines short and long-term needs and help set goals for children and families, monitors the child’s adjustment to foster care, serves as an advocate for the child and family, and provides access to resources and support services. Responsibilities include: communicating the agency’s plan of service for each child to team members and outside officials. Close supervision is received in the form of case plan reviews, one-on-one supervision, and casework conferences. Work is performed under the supervision of the Social Work Supervisor.

Social Worker Levels: I – Entry Level/close supervision/limited caseload progressing to full. II – Progressive authority and responsibility, and full caseload. III – Most responsible level of casework services, equivalent to senior social worker, full caseload.

JOB RESPONSIBILITIES

Ideal candidate will assess the unique strengths and needs of each child and family involved, in consultation with the interdisciplinary childcare team and specialty consultants, as appropriate.

Develops, communicates, and implements an individualized service plan and treatment program which responds to identified needs of each child and natural/foster family; engages the child and family in setting goals, establishing priorities, and implementing service/treatment plans.

Participates in permanency planning; sets goals; conducts regularly scheduled Service Plan reviews; tracks progress toward desired outcomes; actively participates in Family Permanency Conferences; intervene to avoid separation of siblings, when feasible.

QUALIFICATIONS AND REQUIREMENTS

 Minimum requirement: Bachelor’s degree from an accredited four-year college/university in social work, psychology or a related field. Master’s degree in Social Work preferred.

PREVENTIVE CASE PLANNER

Summary of Position

Responsible for assessing child and family needs and implementing preventive services in an effort to avert, when possible, foster care placement and disruption of the family unit. Incumbents evaluate the social,  developmental and resource needs of children and families, develop and implement individualized service plans and when necessary, provide protective and placement services for dependent or neglected children.  Work includes counseling families toward resolving individual and family problems and utilizing agency and community resources to assist families in achieving and maintaining self sufficiency and independence.  Work is performed under the supervision of the Case Work Supervisor.   Close supervision is received in the form of review of case work plans, one-on-one supervision and frequent case work conferences.

Characteristic Duties

Assesses unique strengths and needs of each child and family, in consultation with the interdisciplinary child care team and appropriate specialty consultants.

Develops, maintains and communicates an individualized service plan and treatment program which responds to identified needs of each child and family; shares pertinent information ( i.e., psychological, behavioral, educational) with other team members;  conducts regularly scheduled Service Plan reviews.

Actively engages the child, foster parent,  family and/or guardians in case planning, setting goals, establishing priorities and implementing service/treatment plans; tracks progress toward desired outcomes; initiates solutions to eliminate barriers to goal achievement; share information with the children, youths and their families regarding progress toward case plan goals.

Conducts case contact visits in the home and face-to-face counseling sessions as mandated and as appropriate, based on need;  encourage participation of non-custodial or incarcerated parents, when possible; coordinate visits to correctional facilities; record and document assessment results, case plans, services provided, visits made and progress toward goals.

Authorizes and arranges formal referral and treatment activities and monitors levels of participation and effectiveness; advocates for needed services.

Provides services and training programs which enable heads of families and children within the family to become self sustaining; conduct training and instructional programs on-site at correctional facilities; prepare the child and mother for reunification, should reunification be a permanency goal.

Conducts intake, on-going and discharge planning, as well as aftercare activities as appropriate; support the transition of the child and family into the foster care system, alternate living arrangements and/or independent living programs; provides emotional support related to separation, attachment and loss; maintains  the confidentiality of all information, including assessments, treatments, case histories, files and records.

Provides or arranges for appropriate neighborhood based and other services related to the needs (  i.e., social, medical, educational, vocational, financial, legal, child care, home management and emotional) of the child and family.

Apprizes the child and family of their rights and expedites access to entitlements and benefits.

Promptly addresses all safety and protection needs of the child; ensure  that living environments ( i.e., homes and child care facilities) are safe and developmentally/emotionally suitable.  Investigates and responds to all reports or observations of suspected neglect, abuse or exploitation of children; immediately reports and addresses unsafe or reasonably suspicious situations consistent with actions mandated of Mandatory Reports;  takes precautions to eliminate risks.

Ensures that case management activities, documents, record-keeping and reporting comply with regulatory guidelines and quality standards; prepare and submit required forms ( i.e., UCRs, Court Extensions, monthly statistics, discharge summaries) in a timely manner.

Manages and periodically self-audits case records; observes confidentiality and keeps, progress notes and other required documents updated, accurate and up to Uniform Case Record standards.

Maintains professional and technical knowledge; completes mandated and other professional training to increase knowledge and upgrade skills; maintains appropriate credentials to perform specialty functions of the job.

Participates as a responsible, cooperative member of the interdisciplinary team; contributes to the team effort;  maintains positive relationships with the Edwin Gould constituency, i.e., children, families, caretakers, representatives of governing agencies, colleagues, service providers and the community; interprets the program to service providers and the community; assists in developing community interest.

Performs related or similar duties as required or assigned.

QUALIFICATIONS AND REQUIREMENTS

Education: 

Minimum requirement of a Bachelor’s Degree from an accredited four-year college/university in social work, psychology or a related field.  MSW preferred.

Experience:

At least two years of documented satisfactory experience in counseling working with a child-family population. Must know PROMIS & Connections. Bilingual (Spanish) preferred.

Other Relevant Requirements:

Willingness to travel to and work in various prisons and jails and to make home visits in all boroughs of New York City
